Templates for Paubox Forms and dashboard updates

Paubox Forms now includes a library of pre-built templates. Instead of starting from a blank page, you can pick a template, adjust the fields, and publish a HIPAA compliant form in minutes.

Forms is included with every paid Paubox Email Suite subscription at no additional cost.

Why we built form templates

We built templates because customers told us the hardest part of a new form is the first one. Knowing which fields to collect for patient intake or a consent form takes time, and most teams were rebuilding the same forms from scratch.

Available form templates

There are now 13 templates across five categories: Intake and onboarding, consent and authorization, clinical, patient experience, and operations. Each one comes pre-populated with the fields most healthcare teams need, and shows a field count and an estimated completion time so you can pick the right starting point.

How to use a template

For example, suppose your front desk needs to collect insurance details before a visit. Open Paubox Forms, click Create Form, and choose the insurance verification template. The fields are already there. Edit what you need, then publish.

• New patient intake

• Insurance verification

• HIPAA authorization (release of protected health information)

• Telehealth informed consent

• Pre-visit health screening

• Patient satisfaction survey

To build from scratch, select Start fresh in the template window. 

Save a draft or un-publish a form

We also added a draft mode. You can now save a form without publishing it, so you can prepare and review a form privately before it goes live. If you need to take a published form offline, you can un-publish it at any time. The form keeps its existing submissions and stops accepting new ones until you publish it again.

Manage the forms dashboard

The Forms dashboard is easier to manage too. Forms are now separated into tabs:

• Published

• Drafts

• Archived tabs

Stats at the top of the dashboard show your active forms, total submissions, and submissions from the last seven days.
